Name Change Affidavit for eROC (For Directors)

Template for Name Change on eROC Platform This template is designed for Directors who need to request a name change on the eROC platform with the Registrar General of Companies. Use this template if you have changed your name and obtained a new National Identity Card from the Department of Persons Registration of Sri Lanka. The template requires the inclusion of your new name, old name, and new National Identity Card number. The swearing statement should be adjusted based on your religion.
